The A Medal is a craftable Hardmode Clicker accessory. It spawns a sigil that rotates around the player clockwise; keeping the cursor's position on top of the sigil will fill a gauge below the player, at a rate of 1% for every in-game frame. Dealing damage against an enemy with a clicker will ensure the hit is a critical hit, but reduces the gauge by 10% in turn.
