The :Sirember: is a craftable pet summoning item. It summons a Giant Decapitated Floating Siren Head to follow the player. If the player has the Brutal Carnage buff, the Siren Head has a chance to freeze in place, staring forward, then inflicting Obstructed on the player for a few seconds and returning back to normal.
