The The Only Thing I Know For Real is a craftable Hardmode Spatial weapon that is obtainable after Arbitration has been defeated.
Its best modifier is Unreal.

In Game Description

This weapon has a unique damage type; inherits bonuses from all damage classes .
Hold left click to charge the weapon; the longer the weapon is charged, the stronger the resulting attack will become .
Charged attacks will execute Guntrigger Execution , teleporting to your cursor (in range), granting Invincibility for 2 seconds, and slashing foes around you.
When charging the weapon, movement is disabled.
After use of Guntrigger Execution , you are inflicted with the debuff Impact Recoil for 12 seconds.
Impact Recoil halves the damage of Guntrigger Execution and removes Invincibility upon activation.
Timing the charged attack perfectly will deal 50% increased damage.
Right click to activate Guntrigger Parry for 1 second (12 second cooldown).
During this time, all damage sustained is negated and reflected to foes.
Successfully parrying an attack immediately removes Impact Recoil and grants the buff Jetstream Bloodshed .
Jetstream Bloodshed grants 30% increased damage and lasts until damage is taken or the weapon is no longer held.
At 100 HP or below, Guntrigger Parry cooldown is doubled, but successful parries remove the cooldown.
Additionally, Jetstream Bloodshed 's damage buff is increased to 50% and natural life regeneration is disabled.

Trivia

• The weapon is a direct reference to Samuel Rodrigues (Jetstream Sam)'s sword from Metal Gear Rising: Revengeance, the "VT7 High-Frequency Murasama Blade".
  • The name is also a reference to Sam, being the name of the song that plays during his boss fight.
• The tooltip is a reference to what Sam says at the beginning of his duel with Raiden.
  • Raiden also says this line when handed his sword by Blade Wolf during his final fight with Senator Armstrong.
• When "Jetstream Bloodshed" is active, the instrumental version of Jetstream Sam's boss theme will play; at 100 HP or below, the vocal version will play instead.
