The Sanguine Despair is a craftable Pre-Hardmode Spatial magic weapon obtainable after obtaining a Guide Voodoo Doll on a Hardcore character.
Its best modifier is Mythical.

In Game Description

Attacks with this weapon consume 10 health per cast and follow your cursor, piercing up to three times (Health drain can kill).
Holding this weapon grants the buff Feral Despair , which increases damage by 0.5% per missing HP.
Right click to unleash Surging Vampirism , an erratic bolt of energy that deals damage in a massive explosion upon striking an enemy (30 second cooldown).
Surging Vampirism deals bonus damage based on missing HP (capping at 400%).
Additionally, Surging Vampirism heals based on 30% of damage dealt to the first foe struck.
The explosion of Surging Vampirism heals an additional 5% of the damage dealt while inflicting Mortal Wounds for 8 seconds, dealing damage over time.
Attacks against a foe with Mortal Wounds deal 10% increased damage.
Critical strikes against a foe with Mortal Wounds deal 30% increased damage instead.

Notes

• While this weapon is only obtainable through use of a Hardcore character, once obtained it can be used by any character regardless of it's difficulty.
• An easy way to aquire this is to put a Guide Voodoo Dool in a chest, then make a new hardcore character to pick it up. Once you've aquired the essence, you can put that in a chest and go back to your original character.
