The Severed Head is a Hardmode accessory sold by the Suspicious Skeleton after defeating Daffodil. When equipped, it summons a Decayed Head that follows the player around. Any enemy within 21 tiles of the Head will be afflicted with the Backrooms Decay debuff, which deals 15 damage per second if the target is a regular enemy and 7.5 damage per second if the target is a boss. The debuff will not deal damage to enemies at or below 50% of their maximum health.
