The Amalghoul is a craftable Hardmode accessory that combines the effects of Unholy Grail, The Cross, Samson's Cuffs, and Ghastly Rosary. When equipped, it grants the following effects:

• Restoring mana will summon a Ghastly Orb, with its damage equal to 50% of the restored mana amount. The Orbs will follow the player for 5 seconds before flying away and homing in on nearby enemies. A maximum of 6 Ghastly Orbs can exist at a time.

• Pressing the Accessory Hotkey button will make the player emit Cross Bass waves around them for 5 seconds. The waves are emitted once every 12 frames and deal 60 damage to surrounding enemies. After activating the ability, it will enter a 40 second cooldown.

• Decreases player's damage and movement speed by 10%. Every 15 seconds, a Samson's Cuff will attach itself to the nearest enemy within 28 tiles of the player, prioritizing bosses. The Cuff lasts 5 seconds and causes the attached enemy to take 50% more damage.

• Spawns an orbiting Ghastly Hand around the player every 5.42 seconds, up to a maximum of 5 Hands at once. Taking damage will make all Ghastly Hands home in on the player, healing 15 life upon touching them. Afterwards, no Hands will be able to spawn for 12 seconds.
